IRESSA was approved by the Food and Drug Administration to treat advanced non–small-cell lung cancer (NSCLC) in patients who have already received specific chemotherapy (platinum and docetaxel). IRESSA therapy resulted in tumor shrinkage in a number of patients. However, no studies were done comparing IRESSA alone to placebo (sugar pill) or other cancer treatments to show if IRESSA increased survival or improved cancer-related symptoms. In two large studies, adding IRESSA to chemotherapy for the initial treatment of NSCLC did not show improvement when compared with chemotherapy alone; therefore, IRESSA is not approved in this setting. IRESSA is contraindicated in patients with severe hypersensitivity to IRESSA or to any component of IRESSA tablets. IRESSA may cause fetal harm when administered to a pregnant woman; female patients should be advised not to become pregnant while taking IRESSA. Asymptomatic increases in liver transaminases have been observed; therefore, periodic liver function testing should be considered. Discontinuation should be considered if changes are severe. Eye irritation has been observed in patients receiving IRESSA. All patients should be advised to seek medical advice promptly if they develop severe or persistent diarrhea, nausea, anorexia, or vomiting, an onset or worsening of pulmonary symptoms (ie, shortness of breath or cough, often with low-grade fever), eye irritation, or any new symptom. Increases in International Normalized Ratio (INR) and/or bleeding events have been observed in patients taking warfarin with IRESSA. Patients taking warfarin should be monitored regularly for changes in prothrombin time or INR. Comedication with CYP3A4 inducers or drugs that cause significant sustained elevation in gastric pH may decrease plasma concentrations and potentially reduce the efficacy of IRESSA. Potent inhibitors of CYP3A4 may decrease metabolism and increase IRESSA plasma concentrations.

In clinical trials, IRESSA caused tumor shrinkage in some lung cancer patients whose prior chemotherapy failed or could not be tolerated. Tumor responses are important results in this setting because treatment with two to four or more of chemotherapy regimens had already failed. IRESSA is a 250-mg tablet that is taken once every day, with or without food. IRESSA should be taken at about the same time every day for as long as your physician has prescribed it. No pretreatments are required prior to taking IRESSA.
